Transcript Request Form

Requesting Transcripts

Official transcripts will not be released at any time after dismissal or withdrawal from the program unless the student’s account balance is zero. Transcripts will not be released while students are enrolled unless they are current on any payment arrangements they have made with the school. After withdrawal/graduation, copies of transcripts will be mailed or faxed after receipt of a complete online request and a payment of $5.00 per official transcript. Students must use the form below to request a transcript. There will be no charge for unofficial transcripts.

Transcripts may be delayed in the month of June due to the summer break. If you have an urgent need for transcripts, please complete the form, pay the fee, and contact Kay Freeman at kay.freeman@harrisonvilleschools.org.

For attendance or graduation verification, the coordinator will need written consent from the student (can be by email) to release the information.
